Recycled Parts Request Form - Cut Sheets

These cut sheet forms were developed to improve communication between recyclers and collision repair facilities. The forms can be used to identify the requested parts or sections.

Instructions

1. Use the Cut Sheet Example Form as a reference guide.
2. Download and print out the appropriate form from the list on the right.
3. Fill in all available information in the upper portion of the form on the second page.
4. Circle the exact section required on the diagrams. Use line to indicate the cut location on the side, top, and underbody views.
5. Use the additional "Notes" section to provide any additional necessary information or sectioning specifications.
